Nutrition Evaluations
All evaluations are stand-alone and are the first step toward working together. The evaluation will assist you in determining actionable nutrition and health goals, outline the road map to meet them and offer suggestions for a nutrition plan and any relevant supplements. Continued work together is done in my signature Concierge Retainer Format.
Nutrition Evaluation $225
A 1 hour systems based assessment of your overall health relative to nutrition status. The type and scope of nutrition support and lab work needed will be discussed, basic food and supplement recommendations will be made and goals will be set, along with a 90 Day treatment plan to connect you your tailored health solution.
Comprehensive Nutrition Evaluation $295
A 1 1/2 hour systems based assessment for chronic and/or multiple diagnosis cases, or those specifically seeking nutrition counseling for assessing and managing intestinal permeability, food allergies/intolerances, cognitive challenges, auto-immune or thyroid dysfunction.
The type and scope of nutrition support and functional nutrition laboratory work needed will be discussed, basic food and supplement recommendations will be made and goals will be set, along with a 90 Day treatment plan to connect you your tailored health solution.
Family Nutrition Evaluation $395
A 2 hour systems based assessment for 2 or more family members seeking nutrition support and planning on implementing similar changes in nutrition and lifestyle habits (gluten free, GAPS, etc)
Basic individual assessment will be done as well as individual goal setting for family members. For complex diagnoses, separate evaluations may need to be conducted. This will be determined in your pre-appointment phone consult.
